2024 My Fun Land Summer Camp Waiver

I, _________________________[Parent/Guardian Name], hereby acknowledge that my child, ___________________, will be participating in the indoor activities summer camp organized by My Fun Land. In consideration for allowing my child to participate in the camp activities, I agree to the following terms and conditions:

1. Assumption of Risk: I understand that participation in the camp activities involves certain risks, including but not limited to, minor injuries, strains, or sprains. I acknowledge that these risks are inherent in physical activities and I voluntarily assume all such risks on behalf of my child.

2. Medical Treatment Authorization: I authorize the camp staff to administer first aid and/or seek medical treatment for my child in case of an emergency. I understand that every effort will be made to contact me or the emergency contact listed on the registration form prior to administering any medical treatment, but in the event that I cannot be reached, I authorize the camp staff to act in the best interest of my child.

3. Release of Liability: I hereby release, waive, discharge, and covenant not to sue My Fun Land, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in the camp activities, whether caused by the negligence of the camp or otherwise.

4. Indemnification: I agree to indemnify and hold harmless My Fun Land, its directors, officers, employees, volunteers, and agents from any and all liabilities, damages, costs, or expenses, including reasonable attorney's fees, arising out of or related to any injury or damage caused by my child's participation in the camp activities.

5. Photo/Video Release: I grant My Fun Land permission to photograph and/or videotape my child during the camp activities and to use such photographs and/or videos for promotional purposes, including but not limited to, website, social media, and printed materials.

I have read this waiver and fully understand its contents. I understand that by signing this waiver, I am giving up certain legal rights on behalf of myself and my child.
